For the Birth Stat Designs ONLY I will need the following:
FIRST Name
MIDDLE Name
Birth Date
Birth Weight and Length
Time of birth (including am or pm)
Design # again (please choose carefully and search ALL designs)
Two or three thread colors (or as is, as in example photo)

Colors will be done as shown in photos unless you have requested otherwise.

Please choose design carefully and only after searching thru ALL the designs. I am noticing a lot of people are ordering design 1 but then putting birth stats in the personalization box. Design 1 is the kindergarten design. I will have to cancel your order.

Please check your messages here often! If I have all the information I will send you a proof within 24-48 hours (birth stat versions only), I will assume we are moving forward if I do not hear from you. If I have questions/need info (in order to move forward ex: no weight etc.) and can not reach you via message/email your order can NOT move forward. I will have to cancel your order if I get no response.
